USA, fraternal organizations
     Fraternal and sororal organizations were a big deal from the middle ages until fairly recently.  There are still a lot of them around.  Many of them do admirable service work, a few are weird, a smaller few are creepy.  Many of them organized themselves in a hierarchical structure, some with quasi-military ranks and ceremonies.  These latter were in the habit of awarding medals and badges for all kinds of things.
